Squarespace setup path

1. Upload this app to hosting, then add it to Squarespace with an Embed block.
2. For shared company use, connect the form to a database service such as Airtable, Supabase, Firebase, or a small custom backend.
3. Add employee login or PIN codes before using it for payroll records.
